The 6 Items pack wasnt for the themes. Only the anniversary edition got that and the code is inside the game case not on a receipt.

To get your packs go to the Locker Tab in the main menu. Then down to Show Shop and then Purchase or Redeem. You will see the pack option there and it will say USE under neath it.

My code was on the GameStop receipt. Just put in the code in the PS store. Then fire up the game and go to the locker tab and there's a place somewhere that says redeem GameStop code or something like that. Don't remember exactly what it says. Then exit out of that screen and go to open new packs and boom.
